Year-End Healthcare Form (1095-C)

Important Health Coverage Document

Under current IRS guidance and the Paperwork Reduction Act, employers are no longer required to automatically mail Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to employees.

Form 1095-C provides information about health coverage that was offered to you during the prior calendar year.  While this form is not required to be attached to your federal income tax return, you may still need it for your personal records.

The due date for employers to provide Form 1095-C available to employees is March 2, 2026.  Though we are targeting for the forms to be available by January 31, 2026.

If you would like to receive a copy of your Form 1095-C, you may request one by contacting Human Resources, Eileen Seichepine at eseichepine@wssd.org or 610-892-3470 x 1406. Upon request, the form will be provided to you within 30 days.
