Student Information Form
Each student must have a completed and signed Student Information Form on file in the nurse's office in order to receive nursing services. The parent/guardian’s signature authorizes the following: treatment in the event of an emergency and the release of medical information to staff on a need-to-know basis.
To keep information current, a new student information forms must be completed on line each year. Please go into your students portal at the school they attend to review and update this form with whenever required information. Providing appropriate emergency health care requires access to student information that is current and accurate.Parents who plan to be away from home for an extended period of time are encouraged to notify the school, in writing, of alternate emergency phone numbers and contact persons.
